Electronic disclosure of plan information regarding your health & welfare and retirement benefits


Endeavor Health distributes important information to plan participants by posting electronic copies of those materials online. The documents and disclosures include: Summary Plan Descriptions (SPD)/Plan Documents, Summary of Material Modifications (SMMs) describing amendments to the plans, Summary of Benefits and Coverage (SBC) documents and Uniform Glossary of Terms, Summary Annual Reports (SARs), Participant Fee Disclosures, Qualified Default Investment Alternative (QDIA) Notices and Pension form 5500’s, and Important notices about your rights, including: the Employer Notice of the Health Insurance Marketplace, Medicare Part D Notice of Creditable Coverage, Premium Assistance under Medicaid and the Children’s Health Insurance Program (CHIP), HIPAA Notice of Privacy Practices, Women’s Health and Cancer Rights Act reminder, Special Enrollment Notice, Notice regarding Wellness Program and Protections from Disclosure of Medical Information, and Notice of Your Rights and Protections Against Surprise Medical Bills. Please be sure you review these materials. They contain important information and answer many of your questions about the benefits offered.


How to access electronic benefits plan information

You can access electronic plan information through ServiceNow, located on the Endeavor Health employee intranet. If you have problems logging on to the intranet, please contact the IT Service Desk:

You also have the right to request and obtain a paper version of any electronically posted document at no charge. To request a paper copy, call the HR Service Center at (847) 570-5111.
