Dependent Eligibility Verification

Exact Sciences conducts a health benefits compliance review to ensure that only eligible dependents are covered by our benefit plans.

As part of this process, employees who include dependents on their benefit plans will be asked to provide proof of their dependents' eligible status with documentation such as, but not limited to:


  • Marriage certificate
  • Birth certificate
  • Adoption papers
  • Court order
  • Tax returns
