To promote making a positive impact on your health and wellbeing, SFA offers a Wellness Incentive that gives you the opportunity to earn incentive dollars in your HSA or FSA for 2025.
Wellness Incentive contributions made by SFA, will be made to your Flexible Spending Account (FSA) if you are enrolled in the Cigna PPO medical plan or into your Health Savings Account (HSA) if you are enrolled in either the Choice Plus CDHP or Choice CDHP medical plans.
To be eligible for the full Wellness Incentive, you must complete:
1. Health Risk Assesment: Become aware of ways to enhance your personal health and wellbeing through the completion of your confidential Health Risk Assessment (HRA).
2. Physician Engagement: Engage with your personal physician through an annual wellness visit.
Your personal physician visit is for a full annual physical exam with your regular personal physician. An annual well woman visit with an OB/GYN will count toward meeting the requirement.
Quick visits to a “minute clinic” or similar facility will not count toward meeting the requirement.
Take the Health Risk Assessment
The online Health Risk Assessment is a brief, confidential questionnaire that evaluates your current health and indicates if you are at risk for certain conditions. It also provides recommendations you may take toward better health.
To count towards the 2025 Wellness Incentive, you must complete the Health Risk Assessment between January 1, 2025 and November 30, 2025.

Cigna and Geisinger will automatically report your completion of the Health Risk Assessment to SFA for purposes of the Wellness Incentive.

Complete the Annual Exam
At your doctor visit, a thorough health history will be obtained, biometrics will be tested, and age- and gender-appropriate tests and screenings will be recommended. Your medical information is completely private; SFA never sees personal health information on any Associate or dependent.
An annual physical exam obtained between July 1, 2024 and November 30, 2025 with your personal physician will be counted. Please note, an annual well woman visit with an OB/GYN will count toward meeting the requirement. Quick visits to a “minute clinic” or similar facility will not count toward meeting the requirement. Cigna and Geisinger will automatically report your completion of the physical exam to SFA for purposes of the Wellness Incentive.
