Leave of Absence

When is a leave of absence required?

If you will miss more than three (3) consecutive, regularly scheduled workdays, you must file a leave of absence request. Please contact Unum Total Leave for all leave requests, updates, and return to work confirmations. For faster service and 24/7 access to manager your leave, please login to portal.unum.com. You may also speak to a Unum Total Leave specialist by calling 866-868-6737, 7 am – 7 pm Central Time, Monday – Friday.

How do I return to work?

  • If you are cleared to return to work full-duty without any restrictions, you must confirm your return date with Unum. Amedisys requires a completed Return to Work Form or doctor’s medical release if you were on leave for your own personal medical condition. Login to your Unum account at portal.unum.com to access the Amedisys return to work form.
  • Unum will contact your manager to confirm you can return.
  • Your manager will contact you to schedule your return

If you need an extension for your leave, or if you can only return to work with restrictions, you must contact Unum. You will need to provide Unum with details from your health care provider. You cannot return to work until you have received confirmation from Amedisys.

More time to spend with your family

Amedisys offers the following additional parental and family caregiving leaves.

Two weeks of 100 percent pay for parental bonding leave for both parents and for adoptions. Birthing parents who enroll in STD through Unum for 2024 receive an additional 6-8 weeks of leave at 60 percent pay.

Two weeks of 100 percent pay for family caregiving to care for an immediate family member with a serious condition as defined by FMLA.

Some things to know: Parental bonding and family caregiving leaves must each be taken as one continuous 2-week period. These leaves run concurrent with any available FMLA leave. The remainder of the FMLA 12-week period is unpaid, but you must use accrued PTO during that time.
