Please tell us which document(s) you would like provided as a paper copy and provide the full mailing address we should use to send it to you. You can make this request by:
- Calling the Amedisys Benefits Center at 888-528-7066 and selecting the option for benefits questions,
- Sending an HR Total Rewards Benefits request online (login required), or
- Using the Help Desk app on your tablet to submit an HR Total Rewards Benefits request.
