2025 Open Enrollment

• Your 2024 medical, dental, vision, life insurance, disability, accident, and critical illness coverages will automatically continue in 2025.
• You will NOT be able to participate in an FSA next year.
• You will NOT receive the Amedisys HSA contribution until you contribute at least $100 to your HSA.
  Note: You can enroll or change your HSA contributions at any time during the year.

You can enroll in Amedisys health insurance, life insurance, disability insurance, and other voluntary benefits if you are classified as a full-time, Baylor, or part-time eligible employee and regularly scheduled to work at least 30 hours per week.

You can also enroll these dependents:

• Your legal spouse;
• Your children, under the age of 26; and/or
• Your children over age 26 who are not able to support themselves due to a physical or mental disability.

Dependent documentation

We will ask for copies of documents—such as a birth certificate or a marriage license—to verify that any newly enrolled dependents meet our eligibility requirements. This allows us to continue to provide all eligible employees and their dependents with comprehensive and competitive coverage.

You do not need to provide these documents until after you enroll your dependents. You will receive detailed instructions on how to verify any newly enrolled dependents in February 2025.

• Change in marital status.
• Birth, adoption or placement for adoption of a child.
• Death of a dependent; and/or.
• Change in your work status or your spouse’s work status that affects benefits eligibility.

Call the Amedisys Benefits Center at 888-528-7066 to complete your enrollment.

Here’s what you CAN do:

• Make medical, dental, vision, HSA and Healthcare FSA elections during Open Enrollment. But you won’t be able to change these when you return to work.

Here’s what you CAN’T do:

• Enroll in life, disability or voluntary benefits while on leave. However, you are allowed to drop or keep these coverages during Open Enrollment. You will have the chance to enroll or make changes to these benefits when you return to work.

You must call the Amedisys Benefits Center at 888-528-7066 within 31 days of your return-to-work date if you wish to enroll or make changes to your life, disability and/or voluntary insurance benefits.

If you are on an intermittent leave of absence, you can enroll or make changes to any of your benefits during the Open Enrollment period. Complete your enrollment online or over the phone by calling the Amedisys Benefits Center.
