Hospital Indemnity (EPO companion plan)
Team members enrolled in the Anthem EPO Medical plan are automatically enrolled in the hospital indemnity companion plan with Unum. The hospital indemnity plan is paid for by PDS Health and covers you (and your covered dependents) in the event of a hospital stay.
If you wish to have extra coverage or to enroll family members that are not covered under the PDS Health medical plan, you can purchase the voluntary hospital indemnity plan for you and your eligible family members. If you are enrolled in both of the hospital indemnity plans (employer paid and voluntary), both plans will provide full coverage in the event of a hospital stay.
