Enroll In Your Health and Other Insurance Benefits

You have 30 days from your date of hire to enroll in benefits. PDS Health offers medical, dental and vision insurances, Health Savings Account (HSA), Flexible Spending Accounts (FSA), life and disability insurances, as well as various voluntary insurances you can enroll in. Watch our new hire video to learn more about your options and explore all the benefit programs offered to you on this Benefits Portal. When you are ready to enroll, visit the People Services Center and follow the steps in this Quick Reference Guide (QRG).

If You Don’t Enroll

You must take action within the 30-day window, even if you want to waive coverage. If the window expires, you will be enrolled in the following default coverage:

Regular Full-Time Team Members:

  • Dental – coverage for yourself only (you must enroll eligible dependents)
  • Short Term Disability
  • Company paid Basic Life and Accidental Death and Dismemberment Insurance (AD&D)
  • 401(k) – Automatic deferral at 4% effective the first of the month after completing one month of continuous employment

Regular Part-Time Team Members:

  • Dental – coverage for yourself only (you must enroll eligible dependents)
  • 401(k) – Automatic deferral at 4% effective the first of the month after completing one month of continuous employment

Outside of open enrollment, you may only make changes to your benefit elections if you experience a qualifying life event, such as marriage, the birth of a child, or losing health coverage.

Choose your Contribution and Investment Fund in the PDS Health 401(k) Plan

The PDS Health 401(k) plan allows you to save a certain percent of your pay on a pre-tax and post-tax basis to invest toward your retirement.

  • Contribution: Eligible team members are automatically enrolled in the plan with a 4% 401(k) pre-tax deferral. In addition to your contributions, PDS Health-supported offices offer a discretionary matching contribution of $0.33 on the dollar, up to the first 6% of compensation you defer. The discretionary match will not exceed $1,500 per plan year.
  • Investment: Your contribution will automatically be invested in the Target Date Fund that most closely matches your retirement date.
