Leave of Absence

Family and Medical Leave of Absence

PDS Health is compliant with all state and federal leave laws. As a team member, you may be entitled to unpaid, job-protected leave of absence for the following reasons:

  • Incapacity due to pregnancy and childbirth
  • To bond with a newly born child, adopted child, or a child placed in your home under foster care (within 12 months of birth, adoption, or placement)
  • To care for your own serious health condition
  • To care for an immediate family member with a serious health condition
  • Qualifying exigencies relating to spouse, child, or parent being called to or on active duty status

There are eligibility requirements for both state and/or federal job-protected leave of absence. For further questions, please contact the Leaves team at Leaves@pacden.com. While on an approved leave of absence, you will continue to be responsible for medical insurance premiums at the team member rate until approved leave expires.

Personal Leave of Absence

Personal Leave of Absence may be granted after 30 days of employment if you are not eligible for any other state or federal leave. Approval and duration are at the discretion of management and shall not be approved beyond six weeks. Use of accrued time off may be required during a personal leave of absence where permitted by applicable law.

If approved, you are expected to return to work on your scheduled return date. Failure to return to work on the scheduled date will be considered a voluntary resignation.
